I made the mistake of thinking outside the box.

I was trying to seat and crimp .223 at the same time with marginally different case lengths, the slightly longer cases seem to have crimped too early while seating, causing the shoulders to mushroom out.

I ran one through the sizing die without the decapping stem to see if it would retake shape
